This chapter describes the evaluation process and essentials of clinical assessment in preschool children with ADHD and comorbid conditions. After a careful review of the challenges in the assessment of preschool children presenting with ADHD symptoms, the AACAP and AAP guidelines for preschool ADHD assessment are presented. A detailed description of the evaluation process includes information on scheduling, evaluation setting, preparing the preschooler for the assessment process, child interview, and observation of parent–child interaction. Various components of clinical assessment are discussed, including history taking and specific questions and play interactions with the preschooler to elicit information regarding symptoms of ADHD and comorbid conditions, child observation, and mental status examination. The importance of obtaining information from multiple settings and contexts is emphasized. A table describing examples of caregiver complaints related to each of the specific DSM-5 ADHD criteria is included. The chapter concludes with a discussion of the integration of data to formulate the diagnosis, treatment plan, and recommendations to parents.
