Case 18 introduces a 69-year-old man with a 40 pack-year smoking history who presents with a longstanding history of chronic cough and recent onset of inspiratory stridor (high-pitched breathing sound) and increasing shortness of breath. The patient states that over the past year, he has experienced a progressively worsening cough. Over the past month, he has been awakened from sleep each night with one or two episodes of coughing. The cough is productive of one to two teaspoons of grey sputum, but he denies noticing any blood.
