This chapter reviews the fundamental principles and changing concepts of nerve stimulation techniques and their proper application in the differential diagnosis of peripheral nerve disorders. Despite the simple principles in theory, pitfalls abound in practice with commonly encountered, yet often overlooked, sources of error in stimulating and recording systems. Other areas of concern include spread of stimulation current, anomalies such as Martin-Gruber anastomosis, and phase cancellation mimicking a conduction block. Collision techniques facilitate waveform analysis to resolve a confusion finding. Short segmental approaches uncover a focal lesion better than evaluating across a longer distance. In contrast, studies of a longer segment detect diffuse or multisegmental abnormalities better, increasing sensitivity and decreasing measurement errors.
