Chapter 16 focuses on key milestones in expanding our understanding of the individual components of the pathophysiological processes and how they interact. Cardiovascular disease remains the major killer of people with kidney disease, and up to know attempts to ameliorate this (especially in patients receiving dialysis) using therapies developed for treatment in the non uraemic general population have almost universally been ineffective. New targets should result in novel interventions and we are only just entering the era of robust testing of this expanded envelope of therapeutic possibilities.
