To manage pain, its function and basic physiology need to be understood. Despite the huge advances in our scientific understanding of pain, most people and many health professionals still do not understand it. Many different models of pain have been put forward over the years and our thinking is always constrained by prevailing conceptual models. The biopsychosocial model allows us to understand a person’s experience of their pain in the context of their life, culture, hopes and aspirations, past experiences, thoughts, feelings, and relationships, influenced by information gained both consciously and subconsciously from family, friends, media, and healthcare professionals. Despite the overwhelming evidence for the effectiveness of this approach to someone in pain, especially chronic pain, it is depressingly common (particularly so after magnetic resonance imaging (MRI) scanning of the spine) to see evidence of the damage done to patients by what has been said to them by healthcare professionals.
