Women with genetic predisposition to ovarian cancer have a lifetime risk of ovarian cancer of over 10%. The recommendation in these women is risk-reducing surgery. As a significant proportion decide not to have surgery, major efforts have been made to explore the option of screening. There is no evidence about whether or not screening saves lives. In women at high risk of ovarian cancer, screening with cancer antigen 125 using time-series algorithms rather than cut-offs is being investigated in trials in the UK and United States. Definitive data about whether screening has any impact on mortality will come from the general population trial, UKCTOCS. Women with Lynch and Cowden syndrome have a genetic predisposition to endometrial cancer. In these women, outpatient hysteroscopy seems to be the best screening approach.
