Breast and ovarian cancer are relatively common. It is therefore important to distinguish diagnoses that have arisen due to single gene mutations leading to hereditary breast and ovarian cancer (HBOC) in contrast to those that have occurred sporadically due to non-hereditary factors. Familial risk of breast and ovarian cancer can be seen to occur along a continuum. The greater the number of affected relatives that are closely related to the proband and the younger they are at the time of diagnosis, the higher the level of inherited risk. Guidelines and models are available for assessing risk of breast and ovarian cancer. This chapter discusses the relationship between risk and family history of breast and ovarian cancer, explaining the rationale for risk assessment. Examples are provided of the type of family history that currently equates to average, raised or high risk and the importance of accurate risk assessment is highlighted.
