This chapter begins with a detailed outlook of the preoperative assessment of the operative risk profile, covering topics such as patient selection and risk stratification, preoperative history and examination (i.e. including the Duke Activity Status Index and AHA Exercise Standards), preoperative investigations (e.g. pulmonary function testing, cardiopulmonary exercise testing), and anaesthetic considerations. The chapter continues to describe the principles of perioperative optimization, and various techniques in operative anaesthetic management (e.g. lung-isolation techniques in oesophageal surgery, fluid therapy, and blood transfusion). Post-operative management, including the management of early post-operative complications, and the principles of enhanced recovery, are then discussed in detail.
