Multiple sclerosis causes cognitive impairment in 40–70 per cent of all patients. Cognitive impairment may be present early in the disease and is more severe in those with a progressive course. Speed of processing information, memory, and executive functions are most commonly impaired. Fatigue, anxiety, and depression, common in MS, can influence cognitive performance. A multisystem disconnection syndrome caused by damage to white matter tracts and intrinsic cortical pathology is thought to explain the cognitive decline. Disease modifying treatments appear to have a beneficial effect of cognition as do acetylcholinesterase inhibitors. The role of cognitive remediation is less clear.
