Cardiothoracic surgery involves major disturbances of normal physiology and compromised regional perfusion, especially of the brain, spinal cord, and viscera. Shifts in fluid balance and manipulations of body temperature are often part of the surgical procedure, and complicate the interpretation of blood gas analysis and metabolic management. For this reason, more invasive and advanced monitoring has been considered a routine requirement for optimal patient management. Both central laboratory and point-of-care testing offer useful and important information to guide practice. In this chapter we discuss the underlying principles and clinical value of monitoring oxygenation, acid-base (including hyperchloraemic metabolic acidosis), lactate, glucose and electrolytes.
