Vestibular migraine is probably the most common cause of episodic spontaneous vertigo in young and middle-aged adults. The diagnosis is usually uncomplicated in a patient with recurrent episodes of vertigo accompanied by migraine headache in the absence of cochlear symptoms. In patients presenting with episodic vertigo that is temporally unrelated to migraine headaches, other migraine symptoms may provide the diagnostic clue. Given the heterogeneous clinical presentation of vestibular migraine, the differential diagnosis has to include a broad spectrum of other vestibular disorders, in particular Menière’s disease. Evidence-based treatment recommendations are still lacking but most experts agree that prophylactic migraine treatment is effective.
