A 6-month-old boy presents via the paediatricians with progressively enlarging head circumference. His head now measures 51.5cm, which makes it far above the 95th centile (5th to 95th centile range 41.5–46cm). There is no medical history of note; he was born by ventouse delivery at 38 weeks and is developmentally normal. Examination finds that there is a bulging fontanelle but no loss of upgaze, and the boy seems alert and normal apart from his head, which is clearly enlarged.
