A 3-year-old boy has a 6-week history of motor decline. His parents report that previously he was running and playing normally but now is walking slowly and holding on to objects or people to support himself. His GP arranged a non-urgent paediatric outpatient appointment. However, for the past 2 days the child has been irritable and holding his head. This morning he vomited and so came to hospital.
