A 61-year-old man was referred by the emergency department at 10p.m. He had felt tired after lunch and gone for a walk, but noticed some tingling in his legs so returned home and fell asleep on the sofa. He woke up around 90 minutes later and was unable to feel or move his legs. He was eventually able to call an ambulance and was brought to hospital. His medical history was notable only for hepatitis C infection for which he was under surveillance. On examination he had no discernible power in his legs although his arms were normal. His lower limb reflexes were brisk with upgoing plantars. He had insensate urinary retention with a residual volume of 800mL.
