Efficient urinary storage requires co-operation between the bladder, urethra, pelvic musculature, and peripheral and central nerve centres.

Three sets of efferent nerves (sympathetic, parasympathetic, and somatic) are responsible for bladder relaxation and outlet contraction during urinary storage.

Mechanisms contributing to overactive bladder symptoms may be sensory or motor, and may be neurogenic, myogenic, mixed, or idiopathic in origin.

Stress urinary incontinence results from a lack of urethral coaptation, a defect in support by the external sphincter or pelvic floor musculature, or both.
