The last few decades have been a triumph for proponents of evidence-based practice in cardiovascular diseases. The onus of responsibility to practice in this fashion and to follow the guidelines is on our shoulders as cardiovascular healthcare providers, and it is intrinsic to our credibility as a profession. It would be a tragedy if it is felt that we are relinquishing the responsibility and if the decisions were driven by payers and not by ourselves.
