A 22-year-old female marathon runner was attending the sports clinic for management of bilateral insufficiency fractures of the femoral head, confirmed on MRI. This had not occurred in association with any increase in her usual training programme and she was struggling to return to training. She did not have any past medical history of note and was not on any regular medications. On further direct questioning she reported mild proximal weakness in the upper and lower limbs and on examination was noted to have a BMI of 17.2kg/m2 with normal stature.
