A 49-year-old Caucasian male presented with a 10-week history of bilateral swelling and stiffness of his MCP joints. His hands were stiff for 15 minutes every morning. He had a 10-year history of mechanical back pain and was on the waiting list for left hip replacement. He had had a right total knee replacement at age 40 years. He had previously been a high-level rugby player. Family history was unremarkable. On examination he had tender swelling of the second and third MCP joints of both hands. He had painless restriction of left ankle and subtalar joint movement.
