Chapter 6 covers optimization, and includes papers that discuss Shoemaker's landmark randomized controlled trial on cardiac output and oxygen delivery values exhibited by major surgery survivors, as well as the haemodynamic monitor to guide fluid, or fluid and drug (intrope or vasodilator) therapy, use of mixed venous oxygen saturation (SvO2) obtained from a central venous catheter as the goal of therapy early (recruited in the emergency room) in severe sepsis or septic shock, and how this result awaits confirmation by the international multicentre studies currently being conducted.
