Contrast angiography has been used in routine clinical practice to evaluate coronary and peripheral artery disease. However, angiography provides only indirect information about the vessel wall structure, possibly misguiding operators in catheter-based interventions. In contrast, intravascular ultrasound (IVUS) has been the first widely-applied clinical imaging technology to directly visualize atherosclerosis and other pathologic conditions within the vessel wall. Its ability to image the entire arterial cross-section in real time offers better understanding of the process of atherosclerosis, as well as improvement of the precision of interventional procedures. This chapter reviews current clinical evidence established by IVUS technology, with an overview of basic image interpretation and IVUS-based assessment of coronary lesions.
