A 35-year-old male financial consultant presented to the emergency department with an extremely painful perianal swelling, fevers, and difficulty walking owing to the perianal pain. He had had two previous perianal abscesses over the past decade, but both had settled with antibiotics. He had had an appendicectomy as a child and a right hemicolectomy 8 years previously for a chronic caecal volvulus.
