A 59-year-old man presented with a 4-week history of jaundice, 6kg weight loss, pale stools, and dark urine. He had not experienced any abdominal pain. There was no previous medical history and he was not taking any regular medication. Alcohol intake was 25 standard drinks per week and the patient smoked 40 cigarettes/day.
