Ankylosing spondylitis (AS) is an immune mediated inflammatory process highly associated with HLA-B27 antigen which is four times more common in men. Commonly, AS generally presents in patients younger than 40 years of age with chronic back pain persisting greater than 3 months. AS begins with an inflammatory cascade resulting in bony ankylosis. Potential extraosseous manifestations include acute anterior uveitis, aortitis, heart block, and interstitial lung disease.
