This chapter demonstrates the imaging findings of focal lower cervical spinal cord atrophy that should suggest Hirayama disease in the appropriate clinical setting. The radiologist should suggest imaging in the flexed position, which shows posterior displacement of the dorsal dura and enlargement of the posterior epidural space filled with prominent epidural veins. Early diagnosis may lead to early treatment with a cervical collar, which may arrest the process.
