This chapter presents and discusses the imaging of a patient with a lumbar spinal paraganglioma. The differential diagnoses for the two key imaging features of this case, namely intradural extramedullary mass and prominent thoracic-lumbar flow voids, are further explored. The clinical presentation, histopathology, and management of paragangliomas are briefly discussed. Key teaching points of the case including the most common differential are highlighted.
