This chapter provides a summary of a landmark study in vascular neurology. Does carotid endarterectomy reduce the risk of stroke or death in patients with symptomatic moderate carotid stenosis? Starting with that question, it describes the basics of the study, including funding, study location, who was studied, how many patients, study design, study intervention, follow-up, endpoints, results, and criticism and limitations. The study was the first large North American randomized controlled trial to demonstrate the benefits of carotid endarterectomy for symptomatic patients with 50%–69% carotid stenosis and to describe perioperative risk among the chosen patient population. The chapter briefly reviews other relevant studies and information, and concludes with a relevant clinical case.
