Barriers remain for people with heart failure who require access to supportive and palliative care. This chapter looks at how barriers may be overcome by using an integrated team approach based on individual needs assessment. Thus patient access to supportive and palliative care is triggered by problems experienced by patient and carer rather than depending on an estimate of prognosis.
