David presented at 35 years of age with shortness of breath, right calf pain, and bilateral lung infiltrates after several weeks of generalized weakness. Lung biopsy demonstrated pathologic features of bronchiolitis obliterans with organizing pneumonia (BOOP) and leg ultrasonography detected deep venous thrombosis. Steroids and anticoagulant therapy were initiated. Five days later, David developed massive haemoptysis and severe respiratory failure with dense bilateral pulmonary consolidation. David required intubation for mechanical ventilation. Bronchoscopy confirmed diffuse alveolar haemorrhage....
