Chapter 17 discusses Oxymorphone, which is synthesized from morphine by replacing the hydroxyl group at C6 with a ketone group; as in many other derivatives, there is a double bond between C7 and C8, and a hydroxyl group added to C14. The ketone group makes it more lipid soluble than morphine. Oxymorphone is also the O-demethylated metabolite of oxycodone, but is present in only small concentrations after oxycodone doses.3 It plays a role in analgesia in rats administered oxycodone, but not in humans. Oxymorphone is available as a hydrochloride salt, making it soluble in water. Protein binding is poor.
