As the overall burden of heart failure increases, so too will the proportion of patients surviving to reach the more advanced, symptomatic stages of their disease; however, best practices for management of these stages are currently not well defined. This chapter outlines an approach to pharmacologic treatment of the advanced heart failure patient, identifying gaps in the current evidence base for treatment of this small but growing population. The chapter discusses how hemodynamic profiles can guide selection of therapy alongside specific challenges in advanced heart failure.
