This chapter summarizes neurocognitive research on HIV seropositive individuals who have past or current substance abuse disorders. The work focuses on studies conducted since the introduction of combination antiretroviral therapy. Neuropathological and neurocognitive consequences of HIV disease are described in detail elsewhere in this volume and as such we now focus on neurocognitive correlates of specific drugs of abuse; placing special emphasis on the independent and additive effects of these drugs on the brain and cognition and on implications for clinical management. Finally, we describe promising strategies for treating this vulnerable population, with a specific focus on cognition.
