HIV-associated neuropathies remain the cause of considerable morbidity. The pathobiology of these neuropathies is complex, resulting from many mechanisms through which HIV interacts with the immune and nervous systems. Additional complexity arises because antiretroviral medications can themselves cause or contribute to neuropathy, both directly and through the metabolic complications of long-term treatment. This chapter presents a general conceptual framework for understanding the neuropathies and then discusses specific neuropathies. The concepts discussed are time locking, layering, and non-specific pathology. The specific neuropathies discussed are distal symmetrical sensory polyneuropathy, antiretroviral toxic neuropathies, inflammatory demyelinating neuropathies, mononeuritic multiplex, diffuse infiltrative lymphocytosis syndrome, cytomegalovirus polyradiculopathy, syphilitic polyradiculopathy, human T-lymphotropic virus neuropathy, and statin neuropathy.
