This chapter introduces the q-space formalism, a model free approach to analyze water diffusion that allows the extraction of the displacement distribution function and therefore quantitative microstructural information on the tissue. It describes the applications of this method in the CNS and spinal cord in models of white matter pathologies, and a q-space approach that tries to model the diffusion components in the tissue. This approach assumes that white matter can be represented by a polydisperse array of closely packed impermeable cylinders and thus one can adapt diffusion MRI based methodologies originally developed in the porous media that are based on the q-space formalism. This includes the Composite Hindered and Restricted Model of Diffusion (CHARMED) consisting of an extra-axonal space in which water diffusion is hindered and distinct intra-axonal spaces in which water is trapped or restricted. The chapter also presents an expansion of the CHARMED framework to model also the axon diameter distribution (ADD) (a framework termed AxCaliber). The applications, experimental and theoretical backgrounds of these methods are described and discussed.
