A 67-year-old man presented with confusion and agitation. His wife explained that he had had 10 days of pain and swelling around the left eye and 2 weeks previously he had been refused elective correction of myopia on the grounds that the eye was infected. There was no past medical history, he lived with his wife, was a non-smoker, and did not drink alcohol.
