A 62-year-old man, a life-long nonsmoker with no known chronic pulmonary disease, presented with slowly progressive chronic exertional dyspnea. Autonomic testing showed severe autonomic abnormalities in several domains (the total QASAT score was 37), likely due to small fiber neuropathy. Cerebral blood flow velocity was reduced in supine position and during the tilt due to hypocapnia.
