The tilt test showed marked reduction in cerebral blood flow velocity (CBFv, 46.5%), which was associated with hypocapnia without orthostatic hypotension. There was mild autonomic dysfunction, affecting adrenergic system. There was no evidence of orthostatic hypotension. This case is not diagnostic for multiple system atrophy.
