Chapter 11 discusses plastic changes in the nervous system as the basis for recovery after brain damage. Ischemia initiates a cascade of cellular events ending in cell death. In a region with focal ischemia there is central zone where all cells will die and a surrounding penumbra zone with diminished supply, in which neurons may survive. Although neurogenesis occurs in the adult brain, there is no effective replacement of lost neurons. Neuroplasticity is a prerequisite for recovery of function after damage of nervous tissue. Recovery is due to a learning process driven by continuous efforts to perform meaningful tasks. Infants with early brain damage may in some cases recover remarkably well; in others complex functional impairments become stable and may worsen during the first years of life.
