This case highlights some of the issues surrounding the management of endometrial cancer. The management of stage I disease is contentious, and the roles of chemotherapy and radiotherapy, as well as optimal surgical management, are discussed. Pelvic lymphadenectomy for early-stage disease has been debated strongly. Risk group stratification plays a key role in the decision making for adjuvant treatment. Adjuvant radiotherapy is well established in advanced disease. Its role in early disease is less clear, with a definite move towards more brachytherapy and less external beam radiotherapy. Chemotherapy has a definite role in metastatic disease. Hormonal therapy is useful, particularly in frailer patients. The use of novel agents is being explored.
