This introduction gives the background of the first edition, summarizes its concerns and perspective, and explains how the capabilities, context, and institutional structure of health care have changed since 1974. It indicates the ways in which the Kantian (personalist) perspective of the original text may have a different valence in the current context, and it calls for experimental studies of whether the rigorous method of the randomized clinical trial (RCT) is itself rigorously justified. The core concepts of autonomy, lucidity, and informed concept are introduced. Comparison is made to experimental methods in the social and the hard sciences. The notion of equipoise as resolving the ethical dilemmas of RCTs is discussed.
