Cardiac surgery should be considered in all cases of acute heart failure that is attributed to surgically correctable causes. Surgical revascularization, repair of mechanical complications of myocardial infarction, valve repair or replacement, left ventricular aneurysmectomy (in case of a discrete left ventricular aneurysm), mechanical circulatory support, and heart transplantation represent the main surgical interventions that may be offered in the setting of acute (de novo or decompensated chronic) heart failure.
