A 44-year-old bartender presented with pain and numbness over the trunk and abdomen and numbness over the medial aspects of both arms, thighs and feet and ankles. Two weeks previously he had developed visible cervical lymphadenopathy, fever, malaise, rash, and headache. He was intermittently unsteady but he did not think his limbs were weak. He was homosexual and drank 1 bottle of spirits and smoked 60 cigarettes per day. He had recently moved from Africa to the UK. He was not taking any medication.
